Institute for Therapeutic Genital Organ Excision Surgery
I, the undersigned, have been referred to the IT GOES clinic because my genital organs are inadequate and non functional. I agree to not expect or ask my wife or any female to touch my penis ever again. I am allowed, and even encouraged to do cunnilingus and fellatio as often as I can so I will be prepared for my post surgical outcome. My penis is found to even be too small for standard penile inversion male to female sex change surgery. Therefore I have been referred to IT GOES for the following procedures. I have been advised to avoid masturbation, so that my prostate gland swells for easier surgical identification and removal. These procedures are: removal of right and left testicle, and total penectomy, including penile root and prostate gland removal, scrotal conversion to false labia, and section of colon resection to form a false vagina, re-routing of urethral opening, insertion of breast implants, vocal cord revision to achieve higher pitch voice, and reduction of adams apple protrusion.
I have been informed of risks and side effects, such as infection and death which can be risk for any surgical procedure. I will of course be sterile, I understand I will no longer have the ability to orgasm, but accept that a successful surgical outcome will now allow intercourse which I am not now capable of.
My signature below grants IT GOES rights to schedule and perform these surgeries whenever and wherever and in any order deemed necessary as facilities to perform the surgery become available. Since operating rooms may only be available at diverse locations at unusual times due to need for extreme privacy for the IT GOES surgical team, and without prior notice, I understand that I may have to be whisked away at any time by the team without prior knowledge and grant them permission to do so. I understand this is a permanent document, once signed is non-revocable.
